Preparing records for a specialist review
Start with a brief enquiry. Do not attach medical files to the initial contact form. We explain the agreed handling arrangements before collecting records.
A clear question comes first
Tell the coordinator the existing diagnosis and the decision or question you want the specialist to review. If you are enquiring for someone else, we first establish the accepted authorization.
Commonly requested documents
The exact list is confirmed for your specialty and doctor. Available records may include:
- Existing diagnosis and relevant consultation or discharge notes
- Current treatment plan, medication list and treatment history
- Relevant laboratory results and original reports
- Imaging reports and, when requested, the actual image files
- Pathology reports; slides or digital pathology only if the review specifically requires them
Keep originals and dates
Retain original files and report dates. A screenshot or a translation may not contain the information a specialist needs. We confirm formats, language and missing items with the selected provider.
Use the agreed sharing route
We explain who receives the records, for what purpose and how you can request access or deletion of our copies. Do not post records or identifying details in social-media comments.
